Athletic Forms
Every 9th and 11th grade student participating in an athletic activity must have a physical performed and this form completed by the physician before the first day of practice. The parent completes the front side of the form and the physician the back side.
Every high school student (9-12 grade) participating in an athletic activity (volleyball, cross country, basketball, track, or lacrosse) must have one of these on file with the activities director before the first day of practice.
Waiver and Release of Liability Form
Every secondary student (7-12 grade) participating in any activity (athletic or non-athletic) must have one of these on file with the activities director before the first day of practice. If you have more than one secondary student participating, a single form per family is fine.
